Client Intervention and Assistance

SERVICES OFFERED BY CLIENT INTERVENTION AND ASSISTANCE

Social Workers provide the following services:

  • Intake
  • Information and referral
  • Individual and family counseling
  • Assistance in locating health care services, financial benefits and other community resources
  • Education, training and support groups for caregivers
  • Advocacy
  • Crisis Intervention
  • Case management (assessment, connecting to appropriate services to meet clients' needs, advocacy, counseling)
  • Income Tax Clinic (for CFS and Older Adult Centre Clients)

Social workers provide in-home and in-office assessments to identify needs, facilitate access to information and community resources and develop short and long-term service plans.  Monitoring and follow-up service is provided as long as required.
