Goals of the Home Support Programs

• To promote, achieve and maintain health, well being and personal independence for the client
• To provide emotional and physical support to clients and caregivers
• To prevent illness, unnecessary hospitalization and/or institutionalization
• To provide emotional and or physical support to clients and caregivers
• To enable seniors and adults with disabilities to remain living independently in their own homes

Eligibility

To be eligible for the Home Support Services, clients must live in our programs' service area (University Ave. to Roncesvalles Ave. and Bloor St. to Lake) and should be a senior or adult with disabilities.
