Client Intervention and Assistance
SERVICES OFFERED BY CLIENT INTERVENTION AND ASSISTANCE
Social Workers provide the following services:
- Intake
- Information and referral
- Individual and family counseling
- Assistance in locating health care services, financial benefits and other community resources
- Education, training and support groups for caregivers
- Advocacy
- Crisis Intervention
- Case management (assessment, connecting to appropriate services to meet clients' needs, advocacy, counseling)
- Income Tax Clinic (for CFS and Older Adult Centre Clients)
Social workers provide in-home and in-office assessments to identify needs, facilitate access to information and community resources and develop short and long-term service plans. Monitoring and follow-up service is provided as long as required.
